Position Overview
As a Home Health Physical Therapist, you will:
- Deliver individualized physical therapy services in patients’ homes or skilled nursing facilities
- Evaluate patients and develop customized treatment plans
- Monitor progress and adjust care plans as needed
- Collaborate with physicians, caregivers, and interdisciplinary care teams
- Help patients improve mobility, independence, and quality of life
Qualifications
- Doctorate or Degree in Physical Therapy (DPT or PT)
- Minimum 2 years of clinical experience
- Home Health or Skilled Nursing Facility experience preferred
- Active California Physical Therapist License
- Valid California Driver’s License
- Current CPR certification
- Current TB test and Professional Liability Insurance
- Strong communication and organizational skills
- Ability to work independently and manage a flexible caseload

We offer our clients stand alone Mobile services, stand alone Telehealth services, as well as, a combination package of Mobile and Telehealth services, when it is determined at the client’s initial consultation that is important for the achievement of the client’s goals.
